The Ohio River forms the southeastern border of Illinois for 130 miles, from the Wabash River confluence down to Cairo where it meets the Mississippi. This stretch of the Ohio flows through some of the most rugged and scenic terrain in Illinois, passing beneath the bluffs of Hardin County and the historic river town of Cave-in-Rock. The deep pools and swift current create excellent habitat for catfish, sauger, and bass.

Flathead catfish are the marquee species on this reach, with fish exceeding 40 pounds taken from deep holes and along submerged rock ledges. Sauger fishing below Smithland Dam is productive during fall and winter, and smallmouth bass hold along the rocky banks and wing dams throughout the river. The backwater areas near Metropolis and Shawneetown provide calmer water for crappie and largemouth bass.

Public ramps are available at Metropolis, Shawneetown, Cave-in-Rock, and several smaller access points maintained by the Illinois DNR. The Ohio River's commercial navigation means anglers share the water with barges and towboats, but the fishing rewards are well worth the extra attention required. Fort Massac State Park near Metropolis offers camping and a launch ramp with easy access to productive water.
